Appointment Info

A typical SIWSH appointment is much more intensive and thorough than a normal gynecological visit, with Dr. Williams spending at least three times as long with each of her patients. Patients who suffer from pelvic and vulvar pain or dysfunction need additional time, attention, and tests to determine the best course of treatment. At SIWSH, Dr. Williams strives to help women achieve the intimacy they deserve. Initial Visit

Each new patient is asked to fill out an extensive questionnaire several days before her initial visit so that Dr. Williams has time to review it thoroughly. It allows her to really understand the patient’s concerns and medical history; it’s the littlest details that are most concerning.

During the appointment, which may take 1 – 2 hours, Dr. Williams reviews the questionnaire again with the patient. This is followed by a pelvic evaluation which includes a variety of tests, including a vulvoscopy (examination of the area around the vagina under a microscope), a physical exam, and possible culture to test for infection. After reviewing the results of the tests, Dr. Williams discusses her findings with the patient and shares her recommended plan of care (POC). The POC can include some combination of regular prescriptions, compounded medications, blood work, a physical therapy referral, ThermiVa treatments, or surgery.

Follow Up Visits

The patient is usually seen 10 weeks after her initial visit to monitor progress and to adjust her POC if needed. SIWSH, upon request, provides a detailed follow up report for the patient to take to her personal physician to continue long-term care.

Out-of-Town Patients

SIWSH treats women from all over the country. For those who are traveling, SIWSH offers a single, all-inclusive visit to eliminate the need for multiple trips. Prior to the visit, all blood work and questionnaires will need to be completed, giving Dr. Williams time to understand the patient’s concerns and medical history before seeing the patient. This will facilitate Dr. Williams in creating the most thorough POC at the end of the visit.

Payment

At times, insurance companies interfere in how physicians care for their patients by dictating what is and is not covered, with their bottom line coming before the patients’ best interests. As insurance companies’ reimbursements to medical carriers decline, and expenses for running a medical practice increase, frustrations grow for both patients and physicians. Quality of care, personal relationships, and physician accessibility, to name a few, all suffer.

The appointment cost varies on whether a patient wants an evaluation in ONE or TWO visits. After the initial assessment and plan, patients require approximately two follow up visits, and most can be referred back to their GYN for their continued care. Pre-Visit Patient Forms

After you have made an appointment with SIWSH, please fill out the following three electronic forms. Be sure to note when each form should be filled out.

1) Registration Form

Please fill out the Registration Form within 24 hours of making your new patient appointment. Your appointment will not be confirmed until you complete this form. Please carefully review the cancellation and payment policies that are outlined on this form.

2) Questionnaire Form

Please fill out the Questionnaire Form at least 48 hours prior to your new patient appointment. This form is fairly extensive and should take you about an hour to complete. Please note that you cannot save a partially completed form and return to it later; the form must be completed in its entirety in one sitting. Therefore, please be sure to set aside an adequate amount of time to complete this form.

3) Distress Scale Form

Please fill out the Distress Scale Form at least 24 hours prior to each appointment you schedule with us. Unlike the Registration and Questionnaire forms, this form needs to be filled out prior to every appointment you have with us, not just before your new patient appointment. It should not take you more than a few minutes to complete.

 

Medical Records Request
